PHP - MySQL job

We require an experienced PHP - MySQL developer to build a small form-based widget for a website. The website member fills a form, the data is saved, and the widget displays as a jpg or gif with text info superimposed that updates over time. Some graphics ability would also help in order to improve the visual display. We can show examples of the form and the result on other sites.
