Php job - form upload

I need a php developer to help me out with a small change to a website.

We require the CV file upload field to work, so the following happens:

• User fills out all their details
• attaches their CV, as a pdf, word doc, gif or jpg (No larger than 5mb).
• press send
• they receive autoresponse - I have already done all this

• receive an email with all the details, and at the bottom there is a link to download the CV (If one was uploaded) from the server.

Features
• CV’s get uploaded to a folder called cv-uploads on the server
• The link in the email is an absolute link so someone at the company can see which CV the email relates to.

Also
• Please make the validation only jump up to where the RED text is and NOT all the way to the top of the page. Otherwise people think the form has gone and not understood what has happened.

Also if i found your skills and speed great i will happily talk about some other work i have in the pipeline which requires php knowledge, mysql/sql, creation of forms and validations (some being before xmas).

My main issue is time, my clients want and require quick turn arounds, so much be fast and have a great knowledge of how to achieve just tasks.
