Pdf Contract creator

I am looking for very simple access or some other similar tool which can create pdf contracts.

We have several hotels and we do book groups for these hotels. We need someone to create a file/tool where on the top we can have our company logo and information and below that Hotel information for which group is interested and then group name & address/contact person name their arrival & departure date and total number of rooms that they want. (Simple information). And on the bottom we need two boxes to add Terms & condition and additional notes. We want this tool to fill hotel information automatically when we chose which hotel we are creating this contract for. The final output we need in pdf format.

Please let me know if someone can do this very simple design for us.
