Payment Gateway setup [urgent]

We have a wordpress site. We have created a page in the site named as Online Payments. We want to take payments from our clients, if someone wants to pay us via debit or credit card.

PLEASE NOTE that our site is not an eCommerce site.

We just want a payment form on one page. client should be able to enter their name, reference no. , mobile number, email address, amount to pay, and description. when they press pay now button, they will be diverted to cardsave payment gateway page where they will do the full payment process.

What I want from you?

To design and implement a form on my wordpress page linked to my payment gateway page.

To design the skinning pack. (it has an option that we can customize the layout of the payment page)

What payment Gateway I am using?

I am using cardsave payment gateway.
