Outlook Call Back button

The basic premise of the button, is to create a call back button within microsoft Outlook 2003.

This button would be used when informing other collegues they have a call back for them either marked as Urgent or Non- Urgent

Within the Box, the user would be required to either select drop down options or manually enter data within the box.

Once the compile button is pressed the examples within Example 1 & 2 would appear.

I have attached examples of how i want it to look and work.

Iam looking for it to be done as quick and as cheap as possible.
