One off - Squarespace account management

I have an existing v5 squarespace account. I want to start from scratch with this template http://flatiron-demo.squarespace.com

I want somebody to setup my account and get the template on there ready for me to use. I have a couple of very minor customisations I would like done to the template too if possible.

Finally I want to have my own URL which I already have hosted with 1and1 setup to be my Squaresapce domain. I own www.creativeimmortal.com but I wish to use that for my Squarespace site.
