OCR or manual data entry bank statements from PDF to MS Excel format

approx. 250 pages of bank statements (sample attached), scanned and saved as PDF, need to be optical caracter recognized (or mnually data entered) and saved in MS Excel format with date distributed across multiple columns and rows as shown on the original (so that the accountant can import and process the information on the statements automatically).
1) PLEASE SEND AN EXAMPLE FILE .XLSX OR .CSV BASED ON THE TEST FILE .PDF ENCLOSED (footers do not need to be included)
