Need two simple JavaScript body type calculators for a swimsuit guide.

In short:

What? Two calculators (one for guys, one for ladies) which take three values and output one of four body types.

When? There is very little rush on this.

More Details:

The code for both calculators will be very similar so once the first one is working it will just be a matter of tweaking a few values to make the other one work.

There will be three text fields, on entering each field some text and an image will be replaced. When the form is submitted the image and text will change again to display the correct body type.

Look at the attached image for a wireframe and a better explanation.

I've attached the some of the code from a calculator that would be almost exactly like the one we need.
