Need Quality Audit Database Created

Description: I need an MS Access db created to allow managers complete QA audits on Work Transactions. These transactions need to be imported from a central data base and the manager must presented with a random list of work transactions to perform the QA audit. The Audit form must be able to the change based on type of work. On completion the audit must provide QA score for team member. Team member can then review and provide their feedback comments. One last option is for Senior Manager to review Manager audits.
