Need an FAX ORDER sheet developed and designed.

I need some one to design me an order sheet for my products..
something for this site.. Hobuck.com or this site, blackantpills.com

i just need a sheet people can fax in case they don't know how to order in the computer..

and something I email or market to them for the orders...

i want the users to fill out the fax order sheet and either fax to me or email or mail..

i want to be able to add or delete products on the sheet...
