Need an app

wanting an app that we can use for booking in laptop repairs that come into our shop. At the moment we use duplicate books and write out the details. What were wanting is an app that we can use on a tablet , were we can input the customer data and then print it out so that we can give 1 to customer and 1 to go inside the laptop(so we know what repairs are needed). Once the repair is accepted then want the system to send out an SMS message to the mobile number entered saying that their laptop is in for repair.. I have attached an excel file of the booking form
