MySql website

I want a website where a person logs in to their own personal account where they will store information from 25 or so pages that I give them to answer questions. (questions like What is your birthday? etc) They would be able to update whenever they want and they would go back to the same pages to look at the info that they have stored. On the website they would pay $1.99 via Paypal for one years worth of storage, converted to an app it would take them in since they would pay $1.99 for the app. Would also need a way to notify them when a year is about to run out. I have some (not much) of a website but don't know how to make separate accounts for each user, mysql, etc. etc.

When given the format, I will add the 25 or so pages of questions.
