Modify hosted form

Create a new version of a page using a hosted form.
The page is similar to the following:

Its not the same page, but is very similar and the same hosted form in an iframe. You can't currently see the form and iframe, but it normally shows up on the second page.

- Create new page using hosted form in an iframe or different method if wanted (code provided). The data basically needs to be submitted to the Database, through the hosted form, I will show you existing implementations of this form.

- Use the existing design. Just insert the new form code.

- You need to create a new version of the page with a second hosted form and a link.


- We basically need exactly the same design, exactly the same implementation, but the user needs to be able to choose 2 different "routes". You can see an example here: http://bit.ly/hzDMGu - the user can submit either "commercial" or "domestic" forms. We need to make an option for the user like this.

Please see that the form on the second page is hosted on a different site, the data is passed to the database from the hosted form. We need someone to deploy a second hosted form on the page, with the same layout as the first. We provide the code for the iframe form and show you how it is implemented.

No design work needed.


If this isn't clear - I can explain in detail. It should be a fairly simple job - using existing code, just modifying how it works slightly.
