Mobile website template

I need a fully-qualified programmer with at least 2 years prior experience in:

* PHP
* CSS
* CUSTOMIZABLE WEBSITE TEMPLATES
* MOBILE WEBSITES

My company sells mobile websites through our site and we need someone to create a template for a mobile optimized website that can be customized easily.

This will be a squeeze page template consisting of 3 pages, a top header bar, video, autoresponder form (name + email) and a footer with links to a privacy policy and disclaimer page plus Facebook & Twitter buttons. This is a simple 3-page site. No videos or autoresponders need to be supplied, just directions to where the code for both needs to go (see diagram). There is no need for actual content in the privacy and disclaimer pages either, I will add that. Obviously, must work across ALL mobile devices, including - Smartphone, iPhone, Blackberry, iPad, etc.

I will provide the icons for FB & Twitter. I have also provide you with a plain diagram of what the mobile website should look like.

Once order is completed, I will be the sole owner of this template and neither you nor anybody esle will have any rights to use it, unless with implicit permission from myself.

I will test the site before payment is made = 2 days. I have access to various mobile website testing softwares and of course I will test it on my own server, therefore every function must work properly.

This template should be highly customizable with regards to it's colour scheme and writable text, i.e. be able to choose new background colour, be able to change the colours & text in top header bar and footer, etc. You will also need to show me how to customize the template. Don't worry, I'm quite familiar with HTML and PHP, so will soon grasp your instructions.
