Making text document +jpeg into ebook formats

I would like you to format a very short dictionary style book onto ebook formats (epub for Ibooks on Ipod, Kobo and Sony formats), and Mobi Pocket for Amazon Kindle.
The dictionary is about 5000 words, which I would supply in Word. The only art involved is a [jpeg] for cover art, which I will supply.
In terms of layout, it is very simple and the Word document gives an idea of how it should look. I will also be on hand to communicate with you if you have any questions or suggestions.
I think for someone who knows what they are doing this is an easy and straightforward job.
What does success look like for this job? Well, formatted versions as above which you have tested to make sure they work well and function properly. I would then check them on the readers and that would be it. In other words, I want you to ensure that they are fit for market consumption.

Please propose how you would supply these to me.
There is likely to be repeat work for someone who does a good job, because the work will be reproduced in several languages.
By the way, some of the content is offensive language (it is a humorous book), so whoever applies must not be offended and should have a sense of humour!
