Landing/Splash Page with Video Design & Development {Responsive}

Here are the details of the launch/Splash Page:

****landing page where a video is the central focus (I see that you are a graphic designer. Instead of just posting the video, could the video be displayed in a really beautiful user interface? Perhaps even a custom one if the budget allows?)

***Include a bold header for the video in a beautiful font

***pop up data capture form that appears after the video plays and includes a warning that the viewer has X amount of time to fill out the form or the will miss the opportunity mentioned in the video. Form entry for name, mobile phone numbers (specifically/only), email address, and what can we do for you? A counter will count down and the form will disappear once the countdown is complete.

***Live chat function

***Listen to a Podcast Function

***Include an area for three of our favorite testimonials

***Include a satisfaction guaranteed seal (consistent with the overall design of the page)

***Include one extra page for our services. On the services page include six flat web icons representing each service with a title beneath each icon and an "i" icon for information which when clicked on produces a pop up with more information for that service.

***Instead of separate pages/links for about us, include an area for a small paragraph of text at the bottom of the page.

***Include contact information and social icons in the footer (like your landing page example)

***Include a small mobile number capture form at the bottom of the page with an icon or some way of directing the reader to our free e-book which they receive for signing up (but can receive only after a double opt-in).
