Integrate awber email optin into a horizontal bar under header

Hi Wordpress Wizards,

I'm looking for someone to integrate an aweber optin for into my thesis website.

The code is a bit messy 'cos I had a mailchimp code there, but moved to aweber, and don't know what I can and can't delete.

Here are a few examples of what I'd like the optin bar to look like:

I've made a button that I'd like to use too.

I don't think it's too complicate; I just have no idea how to it!

I'd like it to be done as soon as possible and to look as profesh as possible too!
