I reqire work to be done on my website

I require some changes to: www.energyefficientheating.net
I would like a customer work request form added.
Also email link changed.
Colours changed.
Tab names changed.
Company heading name, logo & phone number changed.
Linked to another www.
Any advice would be great.
A fixed price if possible.

Prefer to talk on the phone about requirements.

Also something is preventing me from adding content.
