I need custom search boxes for my website

I need search boxes similar to the following site http://www.scottbader.com/

All I want is the drop boxes and the search.

This has to be able to be put on any site, so you may have to add some basic css.

What I need

1)I need the way the above site search works, so people can select from the drop boxes and be directed to another page similar to http://www.scottbader.com/speciality-polymers/selector/?row=1

2)It needs to be easy to add products too, and easy for another programmer to understand.
