I need a Word doc transformed into a slick, colourful document to give to subscribers

Information on the business: I'm a copywriter
Industry: Business
Design details: It will become an A4 PDF of several pages (page number not yet fixed but at present in Word form is 5 pages and around 1700 words). This will be delivered electronically when people sign up to my website. I've attached a similar freebie given by a peer in my industry that I really like, however I obviously don't wish to copy them exactly.
Target audience: Established business owners and entrepreneurs in various fields, business savvy, upper end of budget, age 30s-50s.
Content on the front/back side and body: First page will be title and my name, last page will have contact details etc. on it.
Ideas for the visual style: I'll give the successful bidder or shortlist images from my website (work in progress) to show you more. New York, stylish, Mad Men-esque.
Things to avoid: Cartoons
Preferred colors: Website colours are black, grey, yellow and white, but will consider others if in keeping with general tone.
Extra notes: This will be a typical '5 ways to' '7 steps to' kind of document. I've written the content already, I just need to polish it up. I've included some excerpts and links within it that I'd like shortened and tidied up if possible, otherwise I will find a way to do this. I have attached an example. I like the layout and stylishness of this, but would need to adapt this to a different font and colour scheme for my own work.

My budget is limited presently but all reasonable offers will be considered and more work in the future is guaranteed if we work well together (this includes more jobs like this and e-books).
