I need a professional web designer

I need someone to update my website and change 'contact' to 'members'. I want this to open up to a copy I have written followed by a login box underneath that says login/register now. I want the register now to open up to an online form: first name, surname, username, date of birth, city, chosen area: with a drop down menu that allows them to select from a list a will give you. And then. Pay now where they will pay £25 to the company to access the members Area. This should then activate via a link for them to log in and the members page to appear, with 6 boxes they can click on called: creatives, web tutorials, shout-outs, competitions, behind the scenes, and workshops, each of these should lead to a blog page for me to update with information and videos etc. has to look really cool and artistic going with the theme of the company. I don't know much about websites but I need all the information from the registration to come to me some how so I collate it and make I can terminate membership at the end of the year?
