I need a php script designed to allow new members to register on a website

I need a php script designed to allow new members to register on a website using a unique code and pin number.

The user will visit the website registration page and be greeted with the following screen:

Register Your Tag

Enter Code: ____________ [text box]

Enter Pin: ____________ [text box]

SUBMIT

**The code will be a unique combination of letters and numbers. eg. AX674491

The pin will be 4 digits: eg. 1982 - this does not need to be unique (but will have different pins).

****IMPORTANT - I will need to assign the CODE and PIN before user registers on my website.

Unique code relates to an URL - eg http://www.mysite.com/members/AX674491 (use unique code not pin).

If member is not registered for that CODE then page just says...AWAITING USER REGISTRATION.

The user will enter the code and pin and go through to a registration page.

Page 2 - Registration Page

User enters details such as Name, Email, Phone Number, Address, Password etc. - and photo upload.
They must have the choice to display publicly on URL or not (never display password).

eg.
Name ____________ [text box] (Radio Button) Yes/ No - Display Publicly


These details will be displayed on unique code domain: eg. http://www.mysite.com/members/AX674491 (AX674491 is unique code in this example)

**IMPORTANT - I need back end access to add or remove sections on this registration page if I need to.

After Registration
When user has completed registration they are sent an email with details:

This includes email, name, password and..profile address:

eg. http://www.mysite.com/members/AX674491 - (use unique code in domain - AX674491
is unique code in this example)

User Password reminder option can be sent from website if they enter their email.

User sent to thank you page when registered - with thank you at top.

Also, send’s me an email with username and password to say that new member has registered.
