I need a PDF Disclaimer/Waiver/release form

I run an sports company - I need a PDF form that once people buy my services, they download the form acknowledging that they are aware of the risks and in the event of accident, mishap or death, they won't sue.

The company is involved in "Zorbing" and other activities. Details can be given on acceptance of the job but really i'm after something that covers the bases - is presentable and printable with the company name emblazoned on the top
