I need a developer with superior mysql - jquery etc skills

The task:

Create a database in MySQL as defined below

Create an interface using HTML 5, jquery, jquery mobile and php that will allow me to complete a form on a mobile and upload to the database.

Field 6 7 and 8 are images taken on the phone and must be resized client side to a maximum of 75Kb to ensure upload on the GSM network is possible.
