Help drafting a defence to small claims and advice on having the judge changed

I have a silly dispute with United Utilities.

They have never sent me a bill, but they issued me a county court claim for non payment.

They will not settle unless I accept a CCJ. I am not willing to accept a CCJ! Although I have offered to pay the bill if they let me have a copy of it.

Their claim was raised through Money Claim Online. I entered a defence and asked for mediation.

The case was sent to my local Court... and a Judge with whom I have some history has given me 14 days to "write a proper defence or have my defence struck out as it has no substance"

2 Years ago I made an official complaint against this judge for how he dealt with another case I was involved with and my complaint was upheld after a long enquiry. Needless to say this judge was not happy when he had to write to me apologising and was officially censured.

So I need two things:

1) I need help drafting the defence - I know I need certain wordings at certain points but don't know what; I can supply a rough draft, you need to make it formal

2) Point me in the direction of how I can make an application to either have this judge removed from my case, or the matter transferred to another court and draft the wording necessary to make this happen.
