Graphical designer, graphically reformat a report

I need a graphical designer to format the report attached. The graphical designer is expected to make artistically pleasant choices for: background image, fonts, general colour theme, colours for different title levels, borders.

I need the result supplied in HTML/CSS with PNG images. This is a job of making data that is essentially a bit boring still attractive to read.

I would like to see an example or two of your previous work.
