Fix Measurement Price Calculator issue - Woocommerce

I've installed the Measurement Price Calculator extension on a Woocommerce theme but it doesn't seem to be working correctly on the product page.

I'm getting 2 "required length (m)" boxes and 2 "product price" totals on the. Along with a single unit box next to the unit price on the product page.!? (see attached screenshot)

I'm basically trying to sell fabric £x / m and the customer just need to enter the amount of meters required and the extension should work out the total.
