Fix errors on an 80% working PHP contact form

I have a PHP contact form which already works but it needs some adjustments ie. error message added to one required field dropdown list, error messages to appear aligned under whichever field not completed, add cc email field, when the one check box checked, it returns as blank on the received email (should detail the request was checked) and form already includes a simple antispam calculation which doesn't work.
