Finish development of a LiveCycle PDF form.

I have an Adobe LiveCycle form development task that I would like to outsource. The requirement is to convert what is presently a paper based form into what will become a fairly simple LiveCycle form. The necessity for LC rather than Acrobat is for the use of an index to select the blocks of the form that will be visible and hide those that aren't.

You can continue with the form that I have already started developing (attached). The requirement specification and detail is as follows:

A4 Pages = 6 page form (structure and most of first page already developed, 1/2 of the last page already developed)
Header = I've already developed it.
Footer = I've already developed it.
Form body = Final 4.5 pages to be developed.
JavaScript = Replicate existing code for menu functionality.
FormCalc = None required for this form.
Menu = Final items to be added.

I am hoping to find a skilled LiveCycle Designer form developer who is seeking working relationship and projects of varying complexity as I am expecting a number of projects to commence in June this year.

Please provide an overview of your experience with LiveCycle Designer and describe the forms that you have developed for past projects and your estiate of time to complete this task.
