Experienced Magento Developer Required

I need a magento shop customised to collect some additional details from customers at the account request stage.

The shop currently has the following 2 extensions installed:

The shop is a B2B site, and we use the first extension to approve customer accounts and set the customer's price group before they can log in to view the product prices.

At present the customer account request only asks for name, an email address and password. They are then sent an email to advise the account is under a review and once the administrators approve another email is sent to notify the customers of this.

What we need is 7 additional fields adding to this account request stage.

One of the fields will be a 'permission to contact' field. If the customer is happy to agree to be contacted by the company we need them put into a separate database that will be accessed directly by the company to create a mail merge mailing list.
