Employee Contract Required

I need a 12 month employee contract (which then becomes rolling) that I can easily edit and change based on employee credentials.

The contract must be binding under UK law and meet the minimum requirements.

Also to be included:
Bonus scheme (paid quarterly)
Holiday (21 days, 2 week at a time limit unless written permission)
Sickness (and more than 2 days must have a doctors note)
