Email form php script

I have a php script for a simple email form. I need to have a function on this form where the user uploads a file and they can submit it to my email address along with the information in the other fields. I will supply the sample script that i have. I do not want a full redesign of the script, i simply want a browse button inserted in the form along with the php code to drive it.
