Website for ITGS

I want a website for ITGS about a clinic that would require 4 pages starting with home, about us, location, contact us, and for appointments booking. The home page should have a logo on the top and the hospital and doctors name right next to it, under it, A slideshow of the pictures of the clinic from the inside and outside, under it will be information about the clinic. The next page will be the about us, information will be included plus a picture of the doctor and the nurses. The third page, i would like to include a location from google maps that can guide them to the clinic. Contact us page should include opening and closing hours. Last page which would be Appointments, where they can book appointments online.
