Web page design - HTML only. (UK only bids)

I am looking for a UK based web designer to create me HTML pages and form designs as and when required.

I have some initial forms and pages i either need designing or amending. It should not take long as the actual template will stay the same - just content, form fields and options will need creating or amending.

If i am satisfied with the work with an agreeable cost, there will plenty more opportunities for you to carry on designing for me.

The work will be in html format only, as i already have an experienced programmer whil will complete the work.

Please do not offer a bid if you can not meet any of the following terms:

1) be based in the UK so i can contact you via mobile and msn.
2) be available to work within strict and short timescales
3) have experience in web usability and use your own initiative when creating the page, rather than just doing exactly what i state.
4) be able to offer me a sample of what you can do if i send you a page to amend. Contact me for details.
5) offer me your own opinions and suggestions.
6) you should be able to start work straight away.
7) keep me updated on all tasks.
8) confirm your hourly rate to me.

I am expecting a low price bid, as my initial task is just to see if we can work well together, as any future tasks will be paid to you direct.

My main aim is to create new and variant versions of a page i can test to check which of the versions has a better conversion rate.
