Web design: (X)HTML + CSS needed for a site

I am looking for someone to create an (X)HTML + CSS design template for a site I am working on at the moment. In total, there are going to be 2, max 3 pages to design.

The end product is going to be a platform where customers will be able to place orders. I will need a customer registration page plus another page where customers can see the list of their orders. Top right will be a login form (2 fields + login button, horizontally aligned), top left, the logo (we have one already). Below, a toolbar with 2 icons, then the main content and at the bottom, a footer with site navigation links..

I am looking at something simple, but nice to the eye without flash, without Javascript, just pure (X)HTML + CSS. Icons will be provided.

Can you deliver this within max 3 days?
