Web design and CMS

Clinic website must be designed (some Flash included) and added into content management system. I need up to three different design attempts to choose from if I don't like first one and you have to tune the design after first attempt.

Design should be also with AJAX and some other jQuery modern effects.

Please, send some examples of your design work and also please send me a message what will cost the design only as I may take only design from you and let the Indian programmers add the CMS.
