Quiz per menu

I need to create a quiz that can be personalized (the question) andit should be structured lioke this that I attach.

First page:fix
Second page: choose the number of participant (maximum 4)
Third page: choose from two questions (both optional response - the response can be changed as the logo that should be in the background - transparent)
There will be so many third page as the number of partecipants
Forth page response
Fifth page: second response.

All the answer must have a number that must be associated to calculate the result
Depending from the sum of the result it will be shown a kind of wine already load.
