PDF client check in forms

I am looking for a custom built client online check in form, i am a personal trainer and have started to work on line, i wish to have a professional form created using my logo and colour scheme where clients can check in and answer a series of questions so i know how they have been progressing through the week,

Most likely in check box and small text box answers. i wish to send this as a link via email to help save me some time.
