New landing page required for website

need a one page website - landing page

Web domain has already been purchased

Landing page must have a link to download app from App Store and a link to sign up via a form

- For the links, i need them both going to app stores (i will send link to app once i receive) - Tag line above links - 'Download our app now and get notified when your favorite creators drop new content'

- For the sign up form i need it to be titled ' Creator Sign Up' followed by these questions:
1. Name
2. Email
3. Contact Number
4. Platform you create on (drop down that they can choose as many as they need to:- YouTube, Facebook, Rumble, Instagram, Twitter, Tik Tok, Gettr, Other)
