Need a Form collection page added to my existing Wordpress

I have a wordpress website based on Elementor theme.

I need you to design like the example page , same form.

The form data should be collected in the backend. I should be able to download the data via CSV.

If you are familiar with Elementor Pro forms you can do it.

I need Captcha for malware attack checks.
