Lead capture embedding on all blog pages

We are wanting Lead captures on all blog pages. Our website is hosted on shopify under the ‘Warehouse’ theme.

What is needs to do:

1. Be completely editable, we can amend images, CTA
2. Lead capture needs to include: Name, Email Address, Company, Job Title & Country.
Ideally would like it embedding into the blog page.
3. Once the lead capture has been complete the piece of content needs to be automatically sent to the lead (whether it be a PDF, E-book etc), but also a notification is sent to me that a lead form has been completed.
4. The lead captures on the pages need to be unique.
5. The lead capture doesn’t show up unless there is a piece of content attached to it.

Ideal Candidate:

- Proven experience working in shopify
- Examples of lead captures embedded onto websites (this is important)
- Readily available to start the project
