I need a html email template creating (full html email not just artwork)

I require a template html email to be created including the html coding.

This will be a standard format to communicate out to our customer base. The template will then reference specific data fields to show performance over time. I have attached the basis content that needs to be included in the email, obviously this is a basic system generated approach, we need this to look professional etc.

I have also attached a suggestion for a layout, obviously this is a guide rather than a directive ;)
