Change website and functionality

I am looking for a professional, enthusiastic and experienced web designer to change a number of funtionalities on my exiting website.

The website is written in .php web 2.0 and it will need to remain this standard with the use of Ajax on all forms.

I would like to maintain the existing look and feel as it works well.

In summary my business provides Health and Safety Services to clients.

However here are the changes:

I would ike a portal for my clients to log in to, to view/download the work/services i have done for them. So each client will be provided with a secure username/password by me. Please note; this is not self registration process. Only clients who have signed up for my services and request me to do work will be eligible to use a dedicated portal.

The CMS panel should in a very simple manner allow me to upload additions to a clients portal.

I would also like an accident report form that clients can use to record accidents, the form will contain fields, e.g. name of injured, date of injury, what happened, causes, etc. The system should record any completed forms and provide a simple grahical interface to run reports.

I would like to remove tab \"my zone\" and also the current registration process which will not be required.

I would also like to ammend the services provded page.
