Make changes to my current logo, create a favicon

Extra notes: The business name has changed from Faultline Theatre to Faultline Arts - I need to change the logo to match this. I also need to create a favicon, using the 'F' from the logo in a box. I need both in multiple formats so that they can be resized, used as favicons, twitter symbols, incorporated into business cards etc.
I do not know the font used for the current logo. I would like something identical or very similar - must be blocky, modern and highly legible even when the logo is small. Ideally I would like the font too, so I can use the correct font in other contexts.
I do not require a major re-design, just a good updating of the current logo.
