Logo and icons design

I need someone who can create an artistic logo for my iphone application and 3 small images/icons for the inside of the application. The application will present advice on how to tackle with a common health problem. The icons will present everyday items and behaviors. We are looking for simple not overloaded icons that match the iphone style. If you can design the User Interface of the application as well it would be definitely a plus.
