Logo and Headed Paper Design

Hi

I am looking for someone to design a brilliant looking logo and letter head for my company. I have attached a questionnaire I filled in for another quote to do the design as I think it answers lots of the questions needed to complete the job.
As we are a film and production company we want to be able to use the logo as our animated section at the beginning of our productions, so if you are able to animate as well that is a great advantage. Think paramount, universal, or dreamwork s (boy on the moon) the 2-3 second animation at the start of most movies.
