Simple Artwork Design for Water Bottle

The bottle manufacturer has provided an artwork template to work within. See Shiva 500c Template 2016.
Our bottle choices are the white bottle on page 23 and black bottle on page 22. Please follow the artwork guidelines within each for direction on sizing etc.
On the design you will see that we have a text design that wraps all the way around the bottle. At one of the designated "join" points a small gap should be left to act as the start and end edge for the text. This should be overlaid by the Wearwell Text Logo.

We would like to see four versions of the design:
Black bottle with Avenir Regular
Black Bottle with Avenir Bold
White Bottle with Avenir Regular
White Bottle with Avenir Bold.

As per the supplier's instructions "You will need to design & supply the artwork in EPS or Ai outlined format please."

Require Pantone references for colours.
