Each label is 45.7 x 25.4mm
Each label will have a 12mmx12mm QR code, the Text below and space to fill in the information manually

The template needs to allow to upload the 40 different QR codes in a bulk on the page.
Attached is a sample of the QR code. The text needs to match the font as per the logo in the QR code
