I need a DVD cover template design for my VHS to DVD conversion business

I'm about to start converting VHS tapes to DVD and I need a professional looking DVD cover that for the converted films.
My website is www.gingermovies.com so you can get a feel of the logo and design, happy to send images if required.
The most important things to feature on the front and back is the Gingermovies logo, the email (info@gingermovies.com) and the DVD price list (on the back). This can be found at http://www.gingermovies.com/#!vhs-to-dvd/snya8
There also needs to be a space on the spine and on the front (top half ideally) for a white box so that i can write the name of the tape by hand.

Dimensions complete cover
Width: 273 mm or 10.75 inch
Height: 183 mm or 7.2 inch

Dimensions front and back
Width: 129 mm or 5 inch
Height: 183 mm or 7.2 inch

Dimensions side
Width: 14 mm or 0.55 inch
Height: 183 mm or 7.2 inch
