URGENT designer for school reading book

Design details: Primary school reading logs from a company, that offer the opportunity to personalise the front cover.

It's an a5 cover and the only stipulation from my wife is that it has to have the title "Reading Record and Phonics Log" with "Including a Guide for the Reading Helper" the school name (St James Infant School) School mascot (Oscar the dog) and the school sun! I will forward these later.


It doesn't need the space for a name or the phonetics that are there.
